Indication of Moodnor Tablet

Moodnor Tablet falls under a category of medications called tricyclic antidepressants. It is used in the treatment of certain mental/ mood disorders. This medication is primarily used for the treatment of depression, bedwetting, pain due to nerve damage

Pharmacology

Amitriptyline:

• Amitriptyline is a tricyclic antidepressant (TCA).
• It inhibits the reuptake of neurotransmitters, such as serotonin and norepinephrine, in the central nervous system (CNS).
• By blocking the reuptake, amitriptyline increases the levels of these neurotransmitters in the synaptic cleft, enhancing their effects.

Nortriptyline:

• Nortriptyline is a metabolite of amitriptyline.
• It is also a tricyclic antidepressant.
• Similar to amitriptyline, nortriptyline inhibits the reuptake of norepinephrine and serotonin, leading to increased concentrations of these neurotransmitters in the CNS.

So, both amitriptyline and nortriptyline function by affecting the reuptake of neurotransmitters, and nortriptyline is derived from the metabolism of amitriptyline.

Dosage & Administration of Moodnor Tablet

Depression:

• Adults: Begin with a low dose, then increase as necessary to 75-100 mg daily in divided doses or as a single dose. The maximum recommended daily dose is 150 mg.
• Adolescents & Elderly: 30-50 mg daily in divided doses.
• Children: Not recommended for depression.

Nocturnal Enuresis (Bedwetting):

• Child (7 years): 10 mg.
• Child (8-11 years): 10-20 mg.
• Child (Over 11 years): 25-35 mg, 30 minutes before bedtime.
• The maximum period of treatment (including gradual withdrawal) is 3 months.
• Full physical examination and ECG are recommended before further courses of treatment.

Interaction

Interactions with Medications:

• Glimepiride: Used for type 2 diabetes. Regular sugar level monitoring is necessary.
• Linezolid: Used for bacterial infections. Combined use may increase serotonin levels, leading to increased heart rate.
• Ondansetron: Used for gastroenteritis and nausea. Combined use can lead to serotonin syndrome, causing seizures and irregular heart rate.
• Metoprolol: Used for chest pain and high blood pressure. The interaction may cause symptoms like dizziness and low blood pressure.

Interactions with Food:

• Alcohol: Consumption of alcohol with Moodnor Tablet can intensify side effects like dizziness, blurred vision, and sweating.

General Advice:

• Seek Doctor's Advice: Before taking Moodnor Tablet, consult your doctor, especially if you are on medications like glimepiride, linezolid, ondansetron, or metoprolol.
• Monitoring: Regular monitoring of sugar levels is advised when taking glimepiride.
• Serotonin Syndrome: Be cautious about symptoms of serotonin syndrome when using linezolid or ondansetron along with Moodnor Tablet.

Contraindications

If you 

• are hypersensitive to Moodnor Tablet or any ingredient of it.
• had a recent heart attack.
• are scheduled to have surgery soon.
• are suffering from bipolar disorder or schizophrenia.
• are using any other medication for depression.

Pregnancy & Lactation

Nortriptyline's safety during pregnancy has yet to be determined. When there are no other options and the benefit outweighs the danger, nortriptyline should be used during pregnancy.
